bulletPull down the [File] menu and select [New].
bulletFrom the list of templates, choose Feedback Form.  This action creates a new page.   If you want your form on an existing page.  Just select, cut and paste to existing page.
bulletCreating a form using the new page template takes care of setting form field and form properties for you.

bulletThe following form is set to store results in a text file located within the Web Site called feedback.txt.
